Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Task Supply Corp supplies home medical equipment and bills insurance on your behalf. To do that we hold information about your health. This notice explains what we do with it, what we will never do with it, and the rights you have over it.
What we hold
Only what we need to get you your equipment and to get it paid for: your name and contact details, date of birth, insurance details, the prescription and supporting notes from your doctor, the diagnosis codes that justify the equipment, and a record of what we supplied and when.
How we use it
For treatment
We share information with your doctor, your therapist and the people delivering and fitting your equipment, so that what arrives is what you were prescribed and it is set up correctly.
For payment
We send your information to Medicare, Medicaid or your private plan to confirm coverage, obtain prior authorization where it is required, and bill for what was supplied. This is the main reason we hold it.
For our operations
Quality checks, staff training, audits, and the ordinary business of running a supplier — for example confirming a delivery took place, or responding when you tell us something went wrong.
When we may share it without asking you
The law allows or requires disclosure in certain situations. We do so only as narrowly as the law permits:
- when a law, court order or subpoena requires it;
- to public health authorities, including reporting a problem with a medical device to the FDA;
- to health oversight agencies conducting audits or investigations, including Medicare and Medicaid;
- to law enforcement in the limited circumstances the law defines;
- to coroners, medical examiners and funeral directors;
- for organ and tissue donation;
- to avert a serious and imminent threat to health or safety;
- for workers' compensation claims;
- to military authorities and for national security, where the law provides.
When we will ask you first
Your written authorization is required before we may use or disclose your information for marketing, before we may sell it — which we do not do — and for most uses not described in this notice. You may cancel an authorization at any time in writing. Cancelling it does not undo anything already done while it was in force.
Your rights
See and get a copy
You may inspect and receive a copy of the information we hold about you, in paper or electronic form. We may charge a reasonable, cost-based fee for copies.
Ask us to correct it
If you believe something we hold is wrong or incomplete, tell us and we will correct it or explain in writing why we disagree — in which case you may add a statement of your own to the record.
Find out who we told
You may ask for a list of the disclosures we have made, other than those for treatment, payment and operations, going back up to six years.
Ask us to restrict what we share
You may ask us not to share particular information. We are not always obliged to agree — but there is one case where we must: if you pay for an item in full, out of your own pocket, you may tell us not to disclose that item to your health plan, and we will not.
Ask us to contact you differently
If reaching you at a particular number or address would put you at risk, tell us where and how to contact you instead. We will accommodate reasonable requests without asking why.
Get a paper copy of this notice
Even if you agreed to receive it electronically, you may ask for it on paper at any time.
Be told if your information is breached
If your unsecured health information is disclosed in a way that compromises it, we will notify you as the law requires.
Our duties
We are required by law to keep your health information private, to give you this notice describing our obligations, and to follow the terms of the notice currently in force. We may change this notice; a changed notice applies to information we already hold as well as to information we receive afterwards. The current version is always available on this page and at our store.
